All dogs and pups must be current on Distemper, Bordetella (Kennel Cough) and Rabies
(if over 4 months) vaccinations and parasite free to attend session.
(please mail or bring a copy of vaccinations to first class)

Please complete and mail the registration form with payment to: For Pet’s Sake 828 Perkins Dr. #200, Mukwonago, WI 53149. Payment is expected on or before the first class. Credit card payments can only be made over the phone with office personnel in advance of the class starting date (1-888-581-9070 Monday - Friday 10am -2pm). Instructors can not take credit card payments the first night of class.
